What do I need to do to get my medical records transferred to another provider?
You would need to come by our office between the hours of 8 a.m. to 5 p.m., and sign a release of records. Please bring your photo identification with you. We will need 48 hours to get the records processed to be sent to your then new provider.
You may also choose to sign the release of records at the new doctor’s office that you will be going to. They will then send the release to us. Once we receive the release we will forward the records to them.
