Eligible Populations for COVID-19 Pfizer Booster Shots

CDC recommends that the following groups should receive a booster shot of Pfizer COVID-19 Vaccine at least 6 months after completing their Pfizer primary series (i.e., the first 2 doses of a COVID-19 vaccine):

• Are 65 and older
• Are 18 years or older and are a resident of a long-term care facility
• Are aged 50-64, and self-attest to having an underlying medical condition that puts them at high risk for severe COVID-19.
• Are aged 18-49, and self-attest to having an underlying medical condition that puts them at high risk for severe COVID-19 and an individual benefit of having a booster vaccine over their risk of receiving one.
• Are aged 18-64, and self-attest to having an occupational or institutional exposure to COVID-19 (for example, healthcare workers, correctional staff, frontline essential workers), and an individual benefit of having a booster vaccine over their risk of receiving one

People can self-attest that they are in one of these groups. Proof is not needed. 

Pfizer Booster Schedule

Pfizer booster vaccines can be administered as early as 6-months after the individuals second dose. Those eligible can find Pfizer booster shots at their health care provider, pharmacies and other locations where COVID-19 vaccines are available. There is no need for people to go back to the location where they received their original vaccines — most COVID-19 vaccination locations can provide Pfizer boosters.

2. Bring a copy of your insurance card (front and back). Insurance is not required and there will be no out-of-pocket cost to any patient. 
   1. If you have Medicare, please bring or complete your registration form with your standard or base plan Medicare ID  information.
3. (First Dose ONLY) If you scheduled your first dose appointment online through CHA's electronic medical record (EMR), Novel Health, please bring your Novel Health account username and password to your first dose appointment. You will need your account information to schedule your second dose appointment.

Individuals who have already received their first dose, were given a ‘COVID-19 Vaccination Record Card’. The date on the back of that card is NOT an appointment, it is the first day the patient is eligible to receive their second dose:

• Pfizer second dose can be administered as early as 21 days after the first dose
• Moderna second dose can be administered as early as 28 days after the first dose
