Breast & Cervical Cancer Screening -- Free breast exam, mammogram and PAP test for eligible uninsured low income women ages 40-64. Free breast exams and mammogram appointments for women less than age 40 with breast problems. Call (704) 920-1205 for appointments and information. 
 
Care Coordination for Children (CC4C) – CC4C provides care management services to children birth to five years of age who have identified risks and who meet program criteria. CC4C care managers will focus on social, medical and developmental needs. Services may include assistance in making appointments, communicating with providers, making referrals, navigating child care options, community resources and programs, etc. For more information and referral call 704-920-1250.

Communicable Disease Services – Investigation and education related to suspected cases of communicable disease. Call (704) 920-1208 for information.

Dental Health Services – Comprehensive dental care for the family. Medicaid, NC Health Choice, private insurance accepted. Sliding fee scale for Cabarrus County residents. Dental Clinics located in Kannapolis and Concord. Call (704) 920-1070 for appointment.

Southeastern Diabetes Initiative (SEDI) Free Education/Programs - The SEDI program seeks to improve the lives of people with type 2 diabetes by providing self-management education classes, diabetes support groups, nutrition and physical activity programs, health screenings, and outreach activities. Clinical services are also available for type 2 diabetes patients who are eligible. Programs are available for people with type 2 diabetes and those without diabetes. Environmental Health Services – Public pool Inspections, food establishment inspections, permitting for septic tank systems and individual drinking water wells and water testing. Call (704) 920-1207 for information.

Family Planning Services – Exams, lab work, birth control education and birth control methods for women and men. Call (704) 920-1205 for appointment. 
 
HIV—All services provided for free. Walk-in hours 8 am - 4 pm for confidential screenings, pre/post test counseling and education

Immunizations – State required immunizations for children from birth through age 18. For college bound students – MMR and Tetanus. Adults – Tetanus/Diphtheria are available. Routine Tetanus boosters are given/recommended every 10 years, but can be given at 5 years if there is a wound, per guidelines from NC Immunization Branch and CDC. Bring immunization record. Innovative Approaches (IA) - IA project works in partnership with medical providers, community agencies and most importantly, parents of children and youth with special health care needs to improve the service system for special needs (ages birth to 21). Our system improvement work is currently being carried out in Cabarrus, Rowan, and Stanly counties. Pediatric Services - Routine, well-child checkups, primary care & immunizations. Appointments available Monday-Friday 8 am -5 pm. Call (704) 920-1205.

Pregnancy Care Management (PCM) - PCM services focus on coordinating care and services for pregnant women who have identified risk factors. Services may include: assistance in making appointments, communication with providers, making referrals, navigating community resources and programs, etc. For more information and referral call 704-920-1250.

Pregnancy Testing – Testing, counseling and referral services. Fee is $22.00 Call (704) 920-1205 for appointment.

Sexually Transmitted Disease (STD) Services – Testing, treatment and education for persons suspected of having a sexually transmitted disease. Call (704) 920-1205 for appointment.

Strengthening Families Program – Call 704-920-1356 for more information about free participation in a family skills building group. The program is open to families with youth between the ages of 6 and 16. Participants receive free transportation, child care, and dinner at each session. Participant incentives are also provided for adults and youth throughout the program. For participation in this program, a family is defined as a unit with at least one parent/guardian and one minor.

TB Skin Tests & Services - Client must return in 48-72 hours for reading of skin test. Fee is $25.00. Call (704) 920-1205 for appointment.

Vital Records - Deputy Registrar processes certificates for all deaths that occur in Cabarrus County. Call 704-920-1329.

WIC – Supplemental food and nutrition education program for pregnant or postpartum women, infants, and children up to age 5. Breastfeeding information available. Call (704) 920-1204 for appointment.
